We would like
to introduce our new low cost reduced-fee dental plan designed for people who
are not currently covered by any dental insurance. This plan is not
insurance, but offers a discount on our usual and customary fees. All payments for any treatment will be due at
the time of service.
With this
discount plan, there are NO deductibles, NO maximums and No waiting periods. General Dentistry, Crowns, Fillings, Bridges,
Prosthetics, Periodontics, Oral Surgery, TMJ, Cosmetic Dentistry and
Orthodontics are included. (Please note
there are some limited exclusions and are listed on the exclusions/limitations
pages.) This Plan will
offer our member/patients some services at no cost and other services at a 40%
discount. Important:
Please note that if the Care Credit utilized,
the discount will be 30% off the usual and customary fees. (Care Credit is a healthcare credit card.)
Your membership will be
